Job Description:
Kelly Healthcare Resources, a division of Kelly Services, Inc., is currently seeking Complex Case Managers to work at one of the top Insurance companies in the country.
Position responsibilities include identifying adult and pediatric members who can benefit from KMHP’s services and assess needs, develop plan of care, monitor and evaluate member’s progress, serve as advocate for the member and liaison between member and community agencies, facilities, providers, etc.
Job Requirements:
Qualified candidates must be a licensed PA RN, CCM preferred, 3+ years Case Management/clinical experience. Eligible to obtain a CCM within 1 year of employment. Managed Care insurance, home care, Disease Management, assessments and care plan development experience a plus. Good computer, communication and organizational skills required.
